Dr. Michael Mash

PT, DPT, CSCS, SFMA, FMS, Cert. TMM

Hi, I’m Michael, founder of Barbell Rehab.

If you’re a personal trainer or strength coach, you’ve likely worked with clients dealing with low back pain. In fact, up to 80% of people will experience it at some point, and many of them will look to you for guidance in the gym.
That’s a big responsibility.
This course will show you how to confidently navigate those conversations and make smart training decisions when pain is part of the picture. You’ll learn when to keep training, when to modify, and when to refer out.
More importantly, you’ll see exactly how we communicate with clients in these situations, because the entire course is structured as if we’re speaking directly to someone with low back pain.
The goal isn’t just to reduce symptoms. It’s to help your clients stay active, build resilience, and improve their quality of life through training, not avoid it.

What We Cover

Course Modules

MODULE 1 (75 MIN)
Introduction and How Pain Works
In the last decade, our understanding of pain has drastically changed. We now know that pain is a poor indicator of what’s actually happening in the body, and findings like herniated or bulging discs are far more common than most people realize. In this module, you’ll learn how pain actually works, how to communicate this to your clients without creating fear, and how to guide them using simple, evidence-based strategies.

MODULE 2 (55 MIN)
Exercises for Back Pain: A Biopsychosocial Approach
After learning how pain works, you’ll dive into how to find tolerable movements so your clients with low back pain can keep training. Using the follow-along PDF guide, you’ll learn how to identify which movements to limit, which to prioritize, and which exercises to use to keep your clients progressing.

MODULE 3 (30 MIN)
Restoring Mobility and Building Back Stronger
Once pain has settled down, it’s time to rebuild. Avoiding movements only works for so long. In this module, you’ll learn how to reintroduce previously painful movements and safely transition your clients into a structured strengthening program. Many clients finish physiotherapy feeling better, but still lack the strength and confidence to reach their higher-level goals. This is where you step in, and this is exactly how to do it.

MODULE 4 (50 MIN)
Exploring Common Self-Management Strategies
Now that you know how to communicate with clients and guide exercise, it’s time to look at common self-management strategies. In this module, you’ll learn what actually matters when it comes to stretching, foam rolling, posture, heat, ice, and electrical stimulation, and whether they’re worth recommending in the first place. We’ll also break down three simple strategies we use to help clients see meaningful, lasting progress.

MODULE 5 (50 MIN)
Common Low Back Pain Scenarios
In this module, we break down three common low back pain scenarios you’ll see in the gym. First, you’ll learn what’s actually going on with sciatica and how to manage it. Then, we cover the “lifter’s low back tweak” and how to guide clients through it so they can keep training. Finally, we address clients who experience pain with prolonged walking or standing, and how to help them stay active and moving forward.

What do you say when a client tells you:

  • “I can’t squat because of my bulging disc”
  • “My doctor said deadlifts are bad for your back”
  • “I’ve had back pain for 20 years, I can’t lift”
  • “I hurt my back squatting, I’m never doing that again”

Navigating these conversations can  be tricky. In this bonus PDF, we break down these scenarios and show you exactly how to respond in a way that builds confidence, reduces fear, and keeps your clients moving forward.
